For the Healthy Families program, participation begins with an Assessment visit. Anyone,
regardless of employment status, financial status, marital status, age, or gender, can request an assessment as long as they
are expecting a baby or have a baby under the age of three months. Also, if you are a grandparent or foster parent with
guardianship of a baby under the age of three months, you may also request a visit:
